The selected candidate will have at least 1 year practical experience as a Licensed Physical! Therapist. Position offers flexibility of schedule, autonomy, reimbursement of expenses and on-going training. Responsibilities include:
Provide assessment and treatment in patient’s residence. Evaluate and development a home program
Gait training, transfer training, strength and conditioning, modality use, and discharge planning.
Coordination of therapy care with all other Home Care disciplines.
Supervise Physical Therapist Assistant
Document treatment as per skilled care guidelines
Adult and geriatric population
Requirements:
Bachelors Degree in Physical Therapy, Masters preferred
Current DE Physical Therapy licensure
Current driver's license with a safe driving record.
Interpersonal, organizational and documentatio! n skills.
1-2 years PT experience preferred.
